Patients and Method: Patients with congenital heart disease complicated by severe pulmonaryhypertension who were at risk or truly developed postoperative pulmonary hypertension crisis were recruited.Ilomedin was infused postoperatively via a pulmonary artery catheter dwelled during surgery. PulmonarypressureswerecontinuouslymonitoredandtheintensivecarecourseaswellasundesirablereactionsattributedtoIlomedinweredocumented. Results:Therewere21patientswithseverepulmonaryhypertensionrecruitedintothestudy,mostofthemhadprimaryheartdiseaseasventricularseptaldefect.Fivepatientshadpulmonarypressurehigherthansystemicpressures.Postoperatively,18patientsshwednoclearclinicalmanifestationspulmonaryhypertensivecrisisandtheother3hadrepeatedcriseswhichweresuccessfullycontrolledbyincreasingIlomedininfusionrateandotherconventional strategies. There were no death. No significant undisirable effects that might be attributed toIlomedinwasdocumented. Conclusions: Continuous Ilomedin infusion via a catheter dwelled in the pulmonary artery was of higheffectivenessandsafetyinthemanagementofpulmonaryhypertensioninpatientsundegoingcardiacsurgery.
